What form do I use to apply for a new passport?


What do I bring with me to my appointment to apply for a passport?


How do I renew my passport? 

If you can say yes to all of these questions, then you use Form DS-82 and renew by mail. 

I can submit my most recent U.S. passport book and/or passport card with this application. yes or no

I was at least 16 years old when my most recent U.S. passport book or card was issued. yes or no

I was issued my most recent U.S. passport book and/or card less than 15 years ago. yes or no

The U.S. passport book and/or card that I am renewing has not been mutilated, damaged, lost,                           stolen and subsequently found. yes or no

My U.S. passport has not been limited from the the normal ten year validity period. yes or no

I use the same name as on my most recent U.S. passport book and/or card. yes or no

or 

I have had my name changed by marriage or court order and can submit proper certified documentation.

If you answered NO to any of these statements above, STOP - you cannot renew by mail.  You must apply using

Form DS-11 and appear in person before an acceptance agent.
